THE Schoolhouse Family Dental WELLNESS PLAN
A dental savings plan that provides access to the care you deserve at a price you can afford.
The Schoolhouse Family Dental Wellness Plan is a membership-based dental savings plan that has been designed to make dentistry more accessible to our patients. We want to share our God-given gift with as many people as possible without the restrictions imposed by many insurance companies or financing arrangements.
Our Wellness Plan is simple. Members pay an annual fee to receive regular check-ups (exams, cleanings, and x-rays) along with access to reduced rates on other procedures. There are so many benefits to our plan; check them out:

At Schoolhouse Family Dental, we see patients of all ages and varying needs. Therefore, we offer a comprehensive option for our Wellness Plan, which makes dentistry more accessible and offers some great benefits.

Annual In-Office Discount Plan...............$350
Each Additional Family Member..............$250, when purchased the same day
Benefits of our Dental Discount Plan:
- FREE Prophylaxis Cleanings (two times per membership year)
- FREE Periodic Exam (two times per membership year)
- FREE Clinical X-Rays, as necessary
- One (1) FREE Emergency Visit Per Year (usually $178 without insurance)
- 20% Discount on our regular office fees, including all lab-fabricated procedures
The following conditions apply:
- I understand the fee for this Discount Plan is $350 for 12 months
- If I would like to enroll any family member(s), I must pay their 12-month fee of $250 per person on the same day I enroll myself on the plan. A maximum of four (4) family members per family may be added to the plan.
- All scheduled appointments must be kept, or a $75 same-day cancellation fee will be imposed. This fee will not be waived under any circumstances.
- I understand that Scaling & Root Planing, as well as Periodontal Procedures, are not considered cleanings, but I will receive a 20% discount on such procedures.
- I understand that the payment made is strictly for the purchase of the Schoolhouse Family Dental - Dental Discount Plan and NOT for any services being rendered either gratuitously or funded.
- The plan fee is non-refundable. I have received a copy of this page.
